– Make sure that each player completes their form.
– The coach and parent MUST sign the form.
– Email all completed forms BEFORE the event, to Chesterfield Sports Association Executive Director Stuart Duncan at [email protected] 
**Players will not be permitted to play without this being on file with The Beal Center**
